Child Enrollment Form
Please download the Kidzone parent enrollment form and bring it to Kidzone with the enrollment fee. Once we receive the form and monies, we can provide you with the remainder amount of forms to enroll your child.
Health Appraisal
Please download the Health Appraisal and take it to your child’s pediatrician and receive a Health Physical and updated Immunizations. You will need this within 30 days of enrolling your child at Kidzone. You will also need to take this form with you every year when your child has an updated health physical or immunizations.
